
The Outreach Team work with sick children and young people and their families, who need support at home. The Outreach Service covers the Blackpool, Fylde and Wyre area.

- Assess each child or young person who may need care at home.
- Work with the child or young persons GP, district nurse and other services to ensure that the right level of care is provided at the right time and in the right way.
- Provide a listening ear and support.
- Care at the end of a child’s life, usually in the last few weeks working with other professionals.
- Provide information and expertise to families and professionals.
- Visit Brian House children and families when they are in hospital for a prolonged period.
- Provide support at and around the time of a child or young persons death along with other services, such as the Linden Centre.
- Work with others to ensure children are reviewed.

The Outreach Service will be considered for any child or young person who fits Brian House Children’s Hospice criteria. Who require care at the end of their life, or extra support for a period of time, possibly due to major surgery or prolonged hospitalisation or illness.
The decision to accept a child onto the Outreach Service would be made at the regular weekly meeting, which consists of nursing staff, doctors and members of the management team.

The team consists of two full time senior qualified children’s nurse’s with many years experience. This combined experience includes oncology, medical, caring for children with special needs, and caring for children in a hospice.
The Outreach Service will work alongside the Brian House team and other professionals, from health, social, education and other voluntary organisations. Aiming to provide holistic, physical, psychological, emotional and spiritual, individualised care for the children and young people using the Brian House Children’s Hospice Outreach Team.

Anyone can refer to the Outreach Service, whether it be the child/young person themselves, their parents/guardian, or any professional involved in their care. The parent/guardian must have agreed to the referral being made.
